Tony still looks at the front door each time it opens (multiple times.) I know he's anticipating Meadow arriving, but he's facing forward, fixated on everyone who's coming in, and also at the guy sitting at the counter. When the guy goes to the bathroom, Tony is still eyeing him.
At least with the guy getting shot next to Silvio, you see everyone's facial expressions and blood first, it's easy to figure what happened. With Phil, a car tire moves toward his head while he's laying on the ground and the car hops up like it hit a speed bump. With those 2 scenes, you completely understand someone was murdered.

No way he's fixated. Quite the opposite. Yes, he looks up but when you're expecting someone and the bell on the door rings, you're going to look up. Once his family was there, he does not look up anymore. For example, the two black guys walk in, we are shown that they walk in but Tony doesn't even look up at them. When the guy in the USA hat walks in, he looks up, but then immediately goes back to the juke box, not giving any more attention other than that quick glace up.
And Tony doesn't eye the Members Only guy. He doesn't pay him any mind when he walks by to the bathroom.
But hell, that's what is so brilliant about the show. The fact we're even having this discussion!

Glancing at someone is natural and, IMO, doesn't equal keeping an eye on him.
For your scenario to be true, we'd have to believe the members only guy wasn't relevant at all even though Chase uses him prominently in the scene. That's not like Chase.
And we'd also have to ignore the boat conversation. Foreshadowing is rampant throughout the series. That's right up Chase's alley.
